Microneedling for Back Acne Scars

Back and shoulder acne leaves the same pitted scarring and dark marks that facial acne does, on an area you cannot see, cannot reach and cannot treat with the routine you use on your face.

Quick answer

Back acne scarring is treated the same way as facial scarring but over a much larger area, so sessions are longer. Microneedling works on the pitted, textural scars — the indentations left where a lesion healed. Flat brown or red marks are pigment rather than scars and often fade on their own or respond better to a different treatment. Active breakouts must be settled before needling starts.

Why back acne scars get left untreated

Back and shoulder acne is common, often more severe than facial acne because the follicles and oil glands there are larger, and almost always under-treated. You cannot see the area properly, you cannot reach most of it, and the serums and routines people use on their faces rarely make it past the shoulder blades.

What gets left behind falls into two categories that look similar and behave completely differently: true scars, where the skin is physically indented or raised because tissue was lost or over-produced during healing, and post-inflammatory marks, which are flat patches of brown or red pigment where the skin is intact but discoloured. Knowing which you have determines what will actually help.

What to expect

Scars versus marks

This distinction is the single most useful thing to understand before booking anything, because treating one as though it were the other wastes time and money.

Pitted or raised scars

The skin surface itself is changed. These need something that remodels tissue in the dermis, which is what microneedling does.

Flat brown or red marks

Pigment in otherwise intact skin. Many fade over months on their own; others respond to pigment-focused treatments rather than needling.

Active breakouts first

Needling over active, inflamed or infected acne is not appropriate. The breakouts are settled first, which may mean starting with acne treatment.

Large area, long visit

A full back is one of the largest areas we needle, so allow a full appointment.

How the course usually runs

For most clients with back acne, needling is the second stage rather than the first. Getting the active acne under control comes first, both because needling over inflamed skin is inappropriate and because new lesions would simply create new scars behind the ones you just treated.

  • Settle active breakouts first — acne laser treatment or acne facials usually come before needling
  • Tell your technician if you are on or recently finished isotretinoin; it requires a clear gap
  • Wear a loose top to your appointment and home; no backpack straps on a freshly treated back
  • Skip the gym, sauna, hot tub and swimming pool for the rest of the day — sweat on open channels is the main risk here
  • No sun, tanning bed or self-tanner on the back for two weeks either side of a session
  • Keep to your spacing; collagen remodelling is cumulative and works over weeks, not days

What to expect from the result

Pitted scarring improves in depth and definition rather than disappearing. Over a course, the indentations typically become shallower and the overall surface more even, which is what makes scarring less noticeable in raking light — the condition under which it shows most. Deep, narrow ice-pick scars are the most stubborn type anywhere on the body, and we will tell you at the consultation if that is predominantly what you have, rather than letting you discover it after three sessions.

Is it right for you?

Microneedling is mechanical rather than light-based, so it suits every skin tone — which matters here, because post-inflammatory pigmentation after back acne is more common and more persistent in deeper skin tones. It is not appropriate over active, inflamed or infected acne, over keloid-prone skin, or during certain medications. We assess the back properly at a free consultation, including the parts you have not been able to see, and tell you what is scar, what is pigment, and what order to treat them in.

Everything is amazing in here. The staff and their machines. It’s really my fist to have an underarm laser and really works wonders. It gives you zero downtime for every session plus no pain AT ALL! I always love to go… ack here for my session.

Okay, so I finally tried laser hair removal, and I need to share — I’m obsessed 😍 I’ve been tired of the constant shaving routine, especially with summer around the corner. I was kind of nervous at first, but the… was way quicker and less painful than I thought. Honestly, it felt like little zaps on my skin, but nothing unbearable. And the best part? My skin is SO smooth now, and the hair has barely grown back. It’s only been a few sessions, but I’m already seeing a HUGE difference. No more razor bumps, no more shaving every couple of days—it’s such a big change in my skin care routine. If you're thinking about it, do it. I honestly wish I’d started earlier 🙌

FAQ

Microneedling for back & shoulders: your questions

Straight answers from the Oxyderm team. Still wondering about something? Call us. We love these questions.

Can microneedling treat back acne scars?

Yes, for the pitted and textural scars where the skin surface itself has changed. It builds collagen in the dermis, which makes indentations shallower and the surface more even over a course. Flat brown or red marks are pigment, not scars, and are usually better addressed another way.

What is the difference between a scar and a dark mark?

A scar means the skin is physically indented or raised because tissue was lost or over-produced while healing. A post-inflammatory mark is flat — the skin is intact but discoloured. You can often tell by feel, and we will go through your back at the consultation and separate the two.

Can I have it if I still get breakouts?

Not over active, inflamed or infected acne. In most cases the sensible sequence is to settle the acne first with acne laser treatment or acne facials, then needle the scarring left behind. Treating in the other order means new lesions create new scars behind the ones just treated.

How long does a back microneedling session take?

The back is one of the largest areas we needle, so allow a full appointment including numbing time. Your technician will confirm the duration for the exact area at your consultation.

Can I go to the gym afterwards?

Not the same day. Sweat, friction from a gym shirt and shared equipment on freshly treated skin with open micro-channels is the main avoidable risk with this area. Give it a day.

Does it work on darker skin?

Yes. Microneedling does not target pigment, so there is no skin-tone restriction, and that matters here because post-acne pigmentation on the back tends to be more persistent in deeper skin tones. Strict sun protection afterwards is important for everyone and especially so in that case.
