Back Acne: What Order to Treat It In, and Why That Order Matters

Congestion, active breakouts and old scarring on the back need treating in a specific order. Why a back facial comes before scar treatment, and what each step does.

Back Acne: What Order to Treat It In, and Why That Order Matters

Key takeaways

  • Treat in sequence: clear the congestion, settle the active acne, then address the scarring.
  • Microneedling over active breakouts creates new scars behind the ones you just treated.
  • Back follicles and oil glands are larger than facial ones, so congestion there is more stubborn.
  • Much of what people call back scarring is flat pigment, which behaves completely differently.

Why backs are harder than faces

The back has larger oil glands and larger follicles than the face, so congestion is deeper and lesions can be more severe. It is also the one area you cannot see or reach: the serums and routines that work on your face rarely make it past your shoulder blades, and most people have never had a proper look at their own mid-back.

Add the daily contributors — a backpack, a bra strap, a car seat, a sweaty gym shirt, conditioner running down it in the shower — and the follicles have plenty of help getting blocked.

The order that works

  1. Clear the congestion. A back facial does the cleanse, exfoliation and extractions you cannot do yourself. It handles blackheads, build-up and the rough bumpy texture, with no downtime.
  2. Settle the active acne. Where breakouts are inflamed or recurring, acne laser treatment or a series of acne facials addresses them. Severe, deep or cystic acne needs more than a clinic can offer, and we will say so and suggest a medical referral.
  3. Then treat the scarring. Once the skin is calm, microneedling for back acne scars remodels the pitted scarring left behind — 3–6 sessions, 4–6 weeks apart.
Needling a back that is still breaking out is the most common and most expensive mistake here. New lesions simply create new scars behind the ones you have just paid to treat.

Scars or marks? They are not the same thing

This distinction decides whether step three is even necessary.

  • Pitted or raised scars — the skin surface itself has changed because tissue was lost or over-produced during healing. These need collagen remodelling.
  • Flat brown or red marks — the skin is intact and simply discoloured. Many fade on their own over months, and the stubborn ones respond better to pigment-focused treatment than to needling.

Post-inflammatory pigmentation after back acne is more common and more persistent in deeper skin tones, which is one reason microneedling suits this area well: it is mechanical rather than light-based, so it carries no skin-tone restriction. Our guide to scar treatment by type goes through how to tell them apart.

The ordinary things that keep it going

  • Shower after the gym rather than letting sweat sit, and change out of damp kit promptly.
  • Rinse conditioner off your back properly — a genuinely common and completely overlooked cause.
  • Wash gym clothes and bedding regularly, and let your back breathe where you can.
  • Do not pick. Most of the dark marks people want treated started as a lesion that was squeezed.
  • Sunscreen on the back in summer: UV makes fading marks stop fading.

What is a back facial?

The same sequence as a facial — cleanse, exfoliate, extract, treat, soothe — performed on the back and shoulders. It exists because that area gets congested easily and is impossible to treat properly yourself.

Should I treat the acne or the scars first?

The acne, always. Microneedling over active, inflamed breakouts is not appropriate, and treating scarring while new lesions keep forming means creating new scars behind the ones you just treated.

How do I know if I have scars or just marks?

A scar means the skin is physically indented or raised. A post-inflammatory mark is completely flat and only differs in colour. We go through the back at the consultation and separate the two, including the parts you have not been able to see.

Can a back facial clear severe back acne?

No. It handles congestion, blackheads and mild to moderate breakouts well. Severe, deep or cystic acne needs medical treatment, and we would tell you that rather than sell you a series that will not be enough.

Can I go to the gym after a back treatment?

Not the same day. Sweat, friction from a gym shirt and shared equipment on freshly treated skin are the main avoidable risks with this area. Give it a day.

How do I shave or prepare my back beforehand?

For a back facial nothing is needed. If you are having laser on the back, the area must be shaved 24 hours before — tell us when you book if you cannot reach it, because it is one of the most common questions we get.
